The final report on the October 2012 deadly Fly Montserrat crash is yet to be completed and authorities at the Eastern Caribbean Civil Aviation Authority (ECCAA) said it could not give a date as to when this would be completed.

Director General of the Authority Donald McPhail told OBSERVER media there are several outstanding matters to be addressed and ECCAA is still collaborating with other agencies, including the UK Air Accidents Investigation Branch.

Within days of the October 7 tragedy, AV Herald aviation site published a preliminary report, which indicated the plane’s right engine failed and the fuel system was contaminated with water.

Despite already knowing the cause, McPhail said authorities must determine a number of other critical issues that he could not disclose.

VP-MON, crashed shortly after take off from VC Bird International Airport killing the pilot, Jason Forbes, and passengers Annya Duncan of Jamaica and Sandrama Poligadu of Guyana.

A fourth passenger, Michael Hudson of the UK, sustained a fractured leg and several cuts about the body.
